If a GP, practice nurse or other clinician has asked you to monitor your blood pressure at home, please use the forms below.
We recommend patients who need regular monitoring consider purchasing a validated home blood pressure monitor to help manage their blood pressure.
Please complete the monitoring form as advised and return it to the practice when requested.
Home BP Daily – Daily blood pressure monitoring. Please complete and return to gram.bydandclinical@nhs.scot or hand in to the reception desk.
